•  Do not pack heavy equipment in Styrofoam “peanuts.”

•  If warranty repair, enclose a copy of original invoice.

•  Enclose this form in box with equipment to repair.  (Keep a copy for your records.)

•  Use one form for each piece of equipment to be repaired.
